"" Admission Process

Your care at Kuakini Medical Center begins with a visit to the Admissions Department on the day of your scheduled admission. During the registration process, an admissions registrar will 1) obtain or verify your personal and insurance information with you, 2) make a copy of your identification and insurance cards for billing purposes, 3) request your signature on several "Conditions of Admission" forms, and (4) make a copy of your advance directive documents (e.g. living will and durable power of attorney for health care) for the medical record chart. Throughout your hospitalization, you will be required to wear an identification band around your wrist for your safety.

A.M. Admissions
Kuakini offers a pre-admission service that includes a review of required test results, nursing assessment, and patient and family teaching so a patient can be admitted on the same day of the hospital surgery or procedure. Kuakini's Ambulatory Care Services staff provides patients with individualized attention and counseling and helps guide the patient through the pre-admission and A.M. admission process.

To help make your stay as comfortable as possible, we suggest that you bring slippers, a bathrobe and a few toiletry items (e.g. toothbrush, toothpaste, razor or shaver).Please do not bring any electrical items such as a radio or an electric blanket.

In most cases, you will be assigned to a semi-private room. If you wish to have a private room, please let the pre-admissions clerk know in advance. We will do our best to accommodate you, however there are a limited number of private rooms which are generally for patients with special medical conditions.

Location Information
The Admissions Department is located on the ground floor of the Medical Center. If you need assistance with your admission, please come to the Admissions Department or call (808) 547-9789 or in-house extension 8789.

Helpful Reminders
Please leave your jewelry, money and other valuables at home. Kuakini will not assume responsibility for any valuables kept in patient rooms. After an initial assessment of articles is made, any additional articles brought into the Medical Center must be reported to your nurse. We recommend keeping no more than $5 pocket money for buying the daily paper and other small items from the gift shop or vending machines.

Please do not place dentures in tissue paper or on the meal tray. Dentures are easily lost. Ask your nurse for a denture cup and place your dentures in the cup upon removal. Kuakini Medical Center will not assume responsibility for lost dentures or partials.

Parking Validation
If possible, arrange to have someone bring you to the hospital. The Admissions Department will validate your driver's parking ticket on the day that you are admitted to Kuakini. Full parking validation is provided to the patient or the person who assists the patient upon admission. When a patient is discharged, full parking validation is provided  by the Business Services staff; the Admissions Department will provide parking validation for patient discharges during the evenings, nights, weekends and on holidays.
