Geriatric Care Services

Kuakini Geriatric Care, Inc. (KGC), a subsidiary of Kuakini Health System, provides compassionate and comprehensive, personalized care for each of its clients. KGC was founded under Kuakini’s century long Tradition of Caring and its commitment to providing safe, quality health care services for the people of Hawaii. KGC helps its clients reach their fullest potential and works to extend independent functioning of elderly residents by maximizing their physical comforts, while maintaining quality of life and dignity in their remaining years. Programs and services include two satellite Adult Day Care Centers, a residential care home called Kuakini Home, and intermediate and skilled nursing care facilities.

We believe that older adults can improve their life situations with compassionate, comprehensive care. Our goals are to:

     Provide appropriate levels of care to meet the specific needs of each of our clients.
     Prevent mental and physical deterioration through ongoing therapeutic assessments, continuing education, and participation in various social activities, and other types of care for our clients.
     When appropriate, work towards final placement in the client’s own home or other institution that will be most beneficial to their needs.

Adult Day Care

Provides participants with recreational and leisure activities, and educational offerings in a supervised setting; daily program costs include an afternoon meal and snack; provides respite care to support caregivers in the community.

Kuakini Home

Provides 24-hour care including meals, personal care and access to recreational/leisure activities in a supervised home-like setting; an assisted living option providing access to services that help maintain one’s physical, emotional and spiritual well-being, with an emphasis on the promotion of self-determination.

 

 

 

Located on the second floor of Kuakini’s geriatric care facility, the Hale Pulama Mau Building, with capacity to service 34 residents.

 

 

 

Offers supervision and assisted living services 24-hours a day, 7 days a week.

Intermediate Care Facility
(ICF)

A State-licensed facility that provides health monitoring and medical services to individuals with physical, and/or mental conditions requiring care beyond what can be offered within their own home; services include supervised nursing care, therapeutic/leisure activities, dietary services, and access to additional care at the recommendation of a multidisciplinary team of healthcare providers.

 

 

 


Located on the third, fourth and fifth floors of Kuakini’s geriatric care facility, the Hale Pulama Mau Building, with capacity to service 147 residents.

 

 


Offers Intermediate Care Facility services 24-hours a day, 7 days a week.

Skilled Nursing Facility
(SNF)

A State-licensed facility that provides services to individuals who need specialized nursing care, rehabilitation services, and 24-hour health monitoring, as well as to individuals who have a chronic illness and do not require the daily care of a physician.

 

 

 


Located on the sixth floor of Kuakini’s geriatric care facility, the Hale Pulama Mau Building, with capacity to serve 40 residents.

Short-term Respite Services

Respite Services are offered at the care home level only. Kuakini Home provides temporary living alternatives for seniors while their caregivers are on leave. Arrangements can be made for a few days to a few months.

Hospice Services

Additional end-of-life support services are available in Kuakini’s Intermediate Care Facility (ICF) and Skilled Nursing Facility (SNF). These services are provided by specialized caregivers who provide support for both residents and their caregivers.
