Discharge Policy

Kuakini Medical Center is a non-profit hospital providing acute care services. Acute care is defined as daily medical and nursing services that must be provided in a hospital. Throughout your stay, the staff of Kuakini will work closely with you and your family and physician(s) in planning for a smooth and timely discharge from the Medical Center.

If placement into a nursing home, care home, or other community program is indicated, Kuakini's staff will make every reasonable effort to assist with the arrangements. When acute care is no longer required, and when placement arrangements have not been finalized, the patient will be discharged home within three (3) days.

Kuakini recognizes that this policy may cause hardship; however, it is necessary for Kuakini to take this position to conserve and efficiently utilize our financial and human resources in order to continue serving our community. We ask for your cooperation and support of Kuakini's Patient Discharge Policy and appreciate your assistance and understanding.

Care Managers are available to support you in making a safe transition from the Medical Center to home. You can call 547-9189 (in-house ext. 8189) if you have a question about planning for your discharge.

Discharge Prescriptions

For your convenience, discharge prescriptions may be filled by Kuakini Plaza Pharmacy and delivered to your room prior to discharge.

If you would like to order your discharge prescriptions through the Kuakini Plaza Pharmacy, please inform a nurse on duty to help expedite the process. Please be prepared to present your prescription insurance information card, and to pay for your co-pay portion of your prescription order, at the time of your discharge.

Every patient has the right to choose the pharmacy that will fill the discharge prescriptions. Kuakini Plaza Pharmacy is very pleased to offer its services to patients who would prefer to receive their discharge prescriptions prior to leaving the Medical Center.

Regardless of where your preferred pharmacy is located, it is always a good idea to plan for a smooth transition from the hospital to your home. An important step in this transition process is to get your discharge medications at the time of your discharge or soon after your return to home.

Kuakini Plaza Pharmacy is a retail pharmacy that is operated by Kuakini and is located on the Kuakini campus.  It is located on the first floor of the Kuakini Medical Plaza (321 N. Kuakini Street), which is one of the two physician office buildings adjacent to the Medical Center.  Business hours for the Kuakini Plaza Pharmacy: 7:30 a.m. to 5 p.m. on Mondays, 8:30 a.m. to 5 p.m. on Tuesdays through Fridays, and 8:30 a.m. to 12:30 p.m. on Saturdays; It is closed on Sundays and hospital-observed holidays. Kuakini Plaza Pharmacy has an automated telephone system that is available on a 24-hour basis to take prescription refill orders.
